Form does not submit creating a serious problem. Need answer as soon as possible, thansks

    Hi,

    I cloned your form to test it and it seems to be working fine on my end:

    I am able to complete submissions without issue.  I tried a few times and each one went through.

    Is there anything specific we can do within the form to cause it to not be able to submit?  As soon as we can recreate the issues, we can work on getting it fixed.
