Can't add to Facebook group

    I would like to add it to a Facebook group not a page.

    There is no option to embed the form to a facebook group...

    What you can do instead is just post the link/URL of your form http://www.jotform.us/form/52204320056137 to a message to the group.

    Make sure you use secure URL when posting to facebook because facebook require such secure contents.
