Need to send Notification to email address entered on the form

In our "Leave Authorization" form, the user decides if they want to have their supervisor to get a copy of the form. I have it set as a condition question so if they answer "yes", then they have to enter the supervisor's email address. I want the form notification to go to that email address that is entered as well as the person who fills out the form.

You can set conditions for the notification to be sent to the specific email, to get the email value paste the field name destination email field. to send the form to thw submitter you need to add an autoresponder email
