Setting up a table like form?

    Hello, I need to create a form which includes the attached questions, and could use advice as to best way to set up. As you'll note from the screenshot, "Activity," "Tasks Involved", "Who Can Do this Activity", "Collaborators" "Tools Used" and "How do you evaluate it's success" all require free text ("Value of activity" does not - although it can be set up either way), and are dependent upon the activity - of which there are several. I'm not sure if there is a matrix available that allows for free text in all cells, or if I have to use individual text boxes (which could get messy). 

    Suggestions at soonest possible opportunity are appreciated!

    You could use Matrix field that is found under Survey tools if you wish to have the free text input on your form.

    If you on the other hand want to be able to select from a dropdown option in one of the options as well (which seems to me as the case for several options if not all), then I would suggest looking into Configurable list widget.
