how to have the text part of my consent form download with the parts the client fill out.

Trying to get a consent form online so when the client submits the form, it has their signature and other details they provide but also includes the text area of the consent agreement they are signing for.  I have pasted the text part in as html text and it shows in the preview mode, but is not included on the submission I receive.  Suggestions?
