My form is not appearing the same way in view as in builder

I can't seem to make my form have multiple columns. I created something in builder but when I preview it it is totally different! Could you please help me? I have emailed you guys before but no one answered so I would really appreciate some help. When you are looking at the URL that has these forms the one in question is the patient health history form. Please let me know! 

That is because you have injected a custom CSS codes that overrides the field positioning/columns in the form. I've fixed it for you by removing those CSS codes.
