Form fields not responsive in phone view

You guys helped me build an awesome form that was responsive.  In desktop view the name and email would be on 1 line and in phone view the name and email fields would drop to their own line and be 100% width. It has stopped working.  I tried to add the widget "mobile responsvie" but the name and email remain on the same line.

I want them to be on their own line and at 100% width in phone view.

Also, the see price button was perfectly aligned with the fields before. But now the button is a little shorter then the rest of the fields in desktop view.  Anyway to line up the right side button even with the right side fields??
