I use the form for my family services agreement, its a pretty long document of what the company does and what the family is required to do, but when they fill it out and it gets sent to me, its not the whole form, its only the bottom where they fill in sign and date and put address, so I can never prove in a court of law that they signed the agreement, I cant get the whole form they sign. how do I get the whole form and not just the signatures?
I have attached what gets sent to me, and if you look at the service agreement all the wording is gone. please help. thank you 503.619.5565 Julie

By default the headers, text fields are not included in the notification emails. The information that is automatically pulled to your email notification is only the fields from the form. You can however add the text directly to the email notification as the email composer is a full editor that allows to you to insert graphics, create tables, etc. The headers or text on your JotForm can be copied and pasted here in the email content.

You can place a PDF link on your email notification as well.
