Field not showing on form

Good Morning - I'm having trouble in that the line in my form asking for the mother's or guardian #1's full name is not transferring to the live form.

I have attached my website and the screenshot of my template within Jotform. 

Please advise.

Edit the form embedded on your website: http://form.jotform.us/form/52465307966161

Then on the form builder's page, click the "Mother's Or Guardian #1 Name" and

Delete the only condition. Here's how:

Save your form after and that should fix your issue. 

By the way, I have seen some conditions on your account that may have no use on your form already and it okay to be deleted as well.
