How to add page break?

Como posso adicionar Quebra de página ao meu formulário e se posso colocar para os clientes fazerem cadastro no formulário? 

Queria informações sobre isso por favor.

You can add Page Break to your form by dragging and dropping the page break form element to the form layout when editing your form.

Page Break element can be found in Survey Tools tab.

Mas como posso adicionar outra página ao quebra página? Pra ficar a página 1 e 2? 

A segunda coisa seria a seguinte, eu queria fazer cadastros dos clientes do meu app.

I have cloned your form http://www.jotformz.com/form/52450318678662 and added Page Break after "Curso" field. So all fields that are above Page Break will be on page one and all fields that are bellow Page Break will be on page 2.
