No Submit button only Fit Form Login is showing

    I'm having the same issue as this thread; but I am finding it hard to understand how the agent fixed the issue:

    Basically my form allows for 'quick' submit via Fit Form Facebook login (This works great)

     

    or

     

    You will have to fill out a short survey. (I can't place a standard Submit button any more. The fit form appears)

     

    I've tried to add an image manually and then turn that into a submit button; but I can't get it to work.

     

    Pleeaaaaaase help!
