The photo upload is no longer working on mobile devices when iPhones update to iOS9. To see an example, download the Village of Frankfort app.

My colleague KadeJM tested your form http://www.jotform.com/form/50356722587158 on iPhone 6 IOS9 and he was able to upload photo on your form.

Is that the form you are having issues with? Is you form maybe embedded to website?

Also which device are you using iPhone, iPad?

    Looking forward to your reply.

Just had another client say this was an issue. They are not getting the option to open their camera at all -- just select a preexisting photo. Also, a bar that says "Cancel" then shows up across the phone's screen and won't go away.

Just updated my iPhone5 to iOS9 so that I could test the issue first hand. I'm seeing the same thing as the clients -- when you select "Choose File" and bar with "Cancel" shows up and never goes away. There are no other options.

Any help on this ASAP would be greatly appreciated. We've got a lot of smartphone apps using this form and specifically for the image upload capability.

Based on your screenshot, it looks like your form is embedded on a website.

Could you please share with us the link to the page where your form is embedded, so that we can check there. Our colleague Kade from our iOS team has successfully uploaded a file with iOS 9 to your form over the form's direct link:

There may be some scripting conflicts, or some other issues between the form and the page where the form is embedded. I would recommend trying to use our iFrame embed method for embedding your form, and see if that helps.

If you are still unable to upload files even after using the iFrame embed method, please do let us know a link to the page where form is embedded, and we will be happy to check further. Thank you.
