Can you provide us more information regarding this issue? Are you reporting that the notification email for your form is not working? Are you referring to this form - http://www.jotform.com/form/12905934278? I checked the form's recipient for the Notification email and I can see you are sending them to multiple recipient. I checked your form's email history and I can see that the email was sent successfully. You can also check your form's email history on your end.
