Creating a form with multiple upload fields

    I need an option where individual can send me their images, all at the same time, and link it to my email.

    You can use the "File Upload" tool and turn on the "Allow Multiple". Please check this demo form.

    You can also create 12 single File upload tool if you like. Please be reminded to increase the "Max File Size" so that a limit can be set for each upload field.

    You can set up an email notification so that you will be notified via email once a new submission arrives.
