win8 desktop looks good:
http://prntscr.com/8nlr1a
BUT on iPhone - there is no form. Form is just flashing and i can see it only for 1 second, than - it disappear. Screenshot from iPhone(no form at all):

ios embed script opens like:
http://prntscr.com/8nlqwm
Likely Jotform support will recommend use iframe, BUT:
When i use iframe i have problems:
1) no form displaying in win8 desktop (and no erros&conflicts: http://prntscr.com/8nlu4q)
And when i open direct url: http://form.jotformeu.com/form/52561009655354 - i can see form. BUT when i deleting this script - http://pastebin.com/JgnCxhz1 , which comes in code to place from jotform.com - all good, i can see form2) form is overflowing in iPhone, so i cant see all fields in form and submit button also
So.. HOW I CAN EMBED JORTFFORM?? -

Hello rewiaca,
Your understanding it correct. In this case, we would suggest you embed your form using its iFrame embed code.
#1. It could be that there is some code conflict between the iFrame script code and the other scripts available in your webpage. Please embed your form using its iFrame embed code without the script code. I did check your webpage and found that you are only using iFrame embed code already and your form is being displayed correctly.
#2. As we cannot answer multiple questions in one thread, I have moved your second question to a new thread and you will be answered here: http://www.jotform.com/answers/676174
Thank you!

I've pasted iframe code without 2nd line with:

As I check your webpage, the form seems to be displayed correctly now and I was able to submit it normally. I've tried submitting the form using a PC and iPhone simulator and it seems to be working fine.
If you still experience any issues, please let us know. We will be happy to take a look again for you.
Thanks!
