Form is not being displayed correctly in mobile device.

For some reason my form looks perfect on a computer by on Mobile it is all screwy. I believe I have the mobile responsive widget in there. Any advice or help would be appreciated.

I did check your form and I am able to replicate the issue you are having, Your form does not appear correctly in mobile device. This issue is caused because of the form width and the "Horizontal Padding" you have set in your form. Reducing the padding and form width will solve your problem. Please check the screenshot below:
