I have cleared my cache, etc. I really need this to go out by the end of the day today, so if someone could contact me, that would be great. This is starting to be very frustrating. I loved this product up until this week when every thing seemed to be breaking!

Are you on the new layout now? Or, still using the old one. The new layout would look like this:
But both works on my end. After you save the conditional logic, you should also save the form. If that is not working for you, please try logging out and login again.

The new layout is the version that I am losing, I would like to revert to the old layout if this is even possible. I have saved the form, logged out and logged in and it is still sending my tests from the old email notifications that I have since deleted.

The new layout is released to half of our users and will be released to everyone once all possible bugs are found and corrected. If you have any issues with it, please let us know so we can forward the matter to our developers.
I don't think going back to the old layout is possible, but we are doing our best to make it easier for everyone to understand the new interface. This new layout is going to be updated to handle duplicates and possibly show an error so users can correct it directly.
As for saving the conditions, I don't find this issue in your form. As far as I could see it, they are properly configured and saved in your form. But note that you should define the correct email address in the condition, especially if you account email address is different to the recipient email.
