Form does not appear correct when printing from PDF

I am trying to print a form I've created into a single-page easily readable print copy. PDF design is working great until I actually preview it for printing. Then the design is all over the page-nothing like the way I've moved fields around on the page. Is there a way to make it print the way it looks on the design page?

If you would like to print the form itself, I would recommend adding a print button to your form and printing from there:

This should give you the same look when printing as you get when viewing the form.
