    How do I set up a form with a tick box question which on the answer the client has to fill in details.


    Please confirm all staff are PAYE employees:

    [ ] Yes

    [ ] No


    If the client ticks Yes then a box is shown:

    PAYE ref [_______________]

    If the client ticks No then a box is shown:

    We require all staff to be PAYE employees, please explain here [________________]
