I checked your form and indeed, the country selection is not working properly:
I created a quick test form with just the widget and had the same thing occur in my test form. The countries are not shown:
I will forward this to our developers to have a closer look. We will update you here as soon as we have any further information.
For now, you can use a regular dropdown field:
In the options for the field, you can change the special options to "countries" to produce the same list as the widget:

The Country Picker widget has been resolved.
We're sorry it took a while - but we had to get to the root of the issue which had many dependencies.
