I am able to access your forms and the my forms page loads normally as usual. Maybe it was a temporary server issue, would you mind checking it again this time?
May I also suggest to relogin from your account or check it using different browser or ISP if you have any. Perhaps it's a browser or network issue on your end.
If you are still having difficulties accessing the page after that, can you please provide us a screenshot on how it appears on your end? You can upload the screenshot here on forum.
Thanks!

Hi,
First of all, sorry for the inconvenience. Right now, it seems like some of our users are encountering this problem. From our side, I didn't replicate the same problem.
While we are looking for the solution, I suggest you to re-embed your form with iFrame method again. You can find the guide from the link below:
http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
For new "Embed Form" wizard,
I hope this one will fix your problem, please give us a feedback about its last state.
Thanks.
