If i click the link to download, this appear in my screen..

When I logged in to your account, I see that you have 67 forms.

Can you provide us name or ID of your form?

When I've logged in to your account, I've checked your form which is http://www.jotformpro.com/form/51593015577964. I am able to upload file on my end.
