why is my form disabled?

    it is enabled on jotform but says disabled on my website

    Our form Phishing filter have mistakenly disabled the form because it was classified as ponography form. But I manually checked the form and I identified that it was not.

    I have now corrected this and reinstated your form http://www.jotform.com/53036802640145
