How can I see the actual responses entered into my form?

All I see when I view submissions is the submission date and the IP address. Where is all the other information the form has collected?

    Thanks

This probably indicates that all the rest of the fields have been disabled. To enable the fields please click the setting icon located on the top right of the window. You just need to enable any field that you would like to view in the PDF.

Please let us know if this helps.

Sorry, no. That doesn't help. This is what I see.

I have checked the form in question http://www.jotform.us/form/52936733404154

I have found out that it is on your Trash so I have restored it. I have checked the form submission page and was able to see what you mean. However, the reason why you're seeing that is because there are no fields on the form. I have tried to check the revision history of the form and have not found any fields aside from the Fixed format widget.

I hope that answers your question as to why you're not seeing any fields or data on the submission page.

Let us know if you need any further assistance. Thank you.
