Product purchase and donation in same form

 I sent this request earlier today and have not heard anything back

trying to set up a form with 4 conditions/options

1. purchase dinner

2. RSVP for the Event without purchasing dinner or any kind of payment

3. make a donation/sponsor the event with purchase of dinner

4. make a donation and rsvp (no dinner)
