Can you please provide us name or ID of the forms you want to use in Field Manager app?
You can also try to edit forms which you want to use, so they will be at top of My Form folder and it will be easier to find them inside of Field Manager.

Looks like this took care of itself with the passage of time. Apparently it takes 30 minutes or so before a new form propagates to be available in the Field Manager app.
