Form Submissions: How can I get the map to go away

When we have referring provider and primary care provider and it asks for an address, a map comes along with it. We don't need or want the map on there. Is there a way to get rid of it?

There is a 'Show Addresses On Map' option which can be enabled/disabled via Preferences. Please see the next animation.
