My email notifications don't send me all the information

I made a simple form needing the name, email, and results from a checkbox. All I receive in my email notifications when people try to sign up is their name. The other fields are blank. I made the email required, but this still won't send me the information over email. I can view the information saved in JotForm's message responses, but they need to also go to my email correctly.

I check your form and there seems to be error on on the Compose email settings of your Email Notifier. Some of the fields on the email is not included on the form and the email value should be {email30}.

Please check it and make your desired revision.
