my new form keeps saying disabled

    I see that you are collecting credit card information using plain text fields on your JotForm which is not allowed using JotForm. Such forms are considered as phishing forms and accounts associated will be automatically suspended. Please refer to our terms of usage for more information on phishing forms. It is required to integrate one of the payment tools available in order to collect any payments through JotForm.

    These payment tools comply with the PCI/DSS security standards which is mandatory to process the payments. I've now removed the guilty fields on your JotForm and activated your account. You should be able to access your JotForms now.
