Email field is not showing due to wrong conditions

    On the Jotform layout there is a place for customer to enter email address. When you go to actual site that is not showing up as an option.

    Thank you for contacting us.

    I cloned your form and I was able to replicate your issue.However, I could see you have some conditions that are not working due to the field are deleted, now I would suggest you to disable or delete them, I deleted them and then the issue has been solved.
