I checked your form and found that your conditions are overriding themselves. In addition to that, you have 64 conditions and this situation also put some weight to your form.
For instance, you have the following condition for the "if PULSA INTERNET is empty" situation.
Then, you set another for "if PULSA INTERNET is equal to TELKOMSEL INTERNET". In this one, you don't need to hide the fields which have already hidden in the previous condition.
So, the condition for this operation is going to look similar with the following one:
You have these kind of conditions in your form. Please, apply this modifications and lighten your conditions. Also, please don't forget to let us know about results.

I would suggest branching the conditions. Start from the top fields to the bottom. When creating the conditions, you don't need to add a new rule to hide the others when the option is not selected. Just create one for "SHOW" and the fields are automatically hidden by default and will only show up when the show rule is meet.
