My form is blank

    I'm using a free account for my form URL: I've had the form active for about a year or so and I think it's been used as recently as about 6-8 weeks ago. However, when I pulled up the URL the page appears completely blank. When I logged in, the form appears blank in the form editor. I'm not sure why this happened, as it's been quite a while since I've actually logged into the form editor. Before I create a new form, is there any way to recover the original form?

    I've tried cloning your JotForm to my account and see that there are no fields available in the form. The previous edited versions shall be available at Form revision history so that can revert your JotForm to the last working version. Please refer to the following guide that can help you in accessing Form revision history.

    Please try reverting to the last working version available in history and see if the form is recovered. I also notice that there are no submissions available for this JotForm.
