Can I create a login form, even if nobody will submit it?

I understand that there is a problem with password in the forms. but i dont want anyone to fill my form, can I make something like this? (in the pic)

It is not possible to create login forms on JotForm. This is considered phishing and results in the suspension of the account. So if you crate login form, even if nobody submit it, our phishing system will detect it and it will autosuspend your account.
