I do not have Mac to test your form, but I have tested it on my PC in Google Chrome, Mozilla Firefox and Internet Explorer and form is shown properly.
I noticed that you are using script code for embedding form to your website. It is possible that there are some script conflict between scripts from your website and JotForm scripts. We recommend using of iFrame code for embedding forms. You can try to replace script code with iFrame code to see if that will solve your problem.
