    Are you using any WordPress plugin to embed the form on your website? Could you try embedding the form using the method provided in the guide below :

    http://www.jotform.com/help/4-Adding-Forms-to-Wordpress-org-version

    If you still experience the issue, please try embedding the form using iframe embed method.

    It looks like that form is currently embedded with oEmbed method and missing 'allow-forms' permissions.

    Can you please try to embed your form as shown in below video to see if that helps?
