My form has been disabled

    I just was looking for a Contact form generator , and i found this website , it's really great website!

    But the problem .. it says

    Form not found
    This form is disabled.
     

    Please Help me as fast as you can!

    After reviewing your form I noticed that your forms are asking for credentials. This violates our terms and is the reason your forms are disable. Please log into your account and remove the USERNAME AND PASSWORD fields. Once removed your forms will be view able again.

    Hi,

     

    I was able to see that the username and password field are now removed in your form. I have activated your account. You can now access your forms.
