How to create an order form ?

I need a form that sponsors can fill out and select a variety of sponsorship levels, and/or more than one.  I'm not sure if I should select a Product Order Form, some sort of Payment Form or a Point of Sale form.  I need them to be able to select which they want, the quantity, and then give details in a comments section.

Any guidance would be greatly appreciated.
