Preview of form is different to that shown in editor

I have edited a standard template to add extra fields and saved it, but when I go to preview these field do not appear. 

I am assuming that you are referring to the form: 53612458626358. I have just cloned your form and was able to add and remove fields. When I opened the form was properly shown with does fields.
