I have tested one of your forms in Google Chrome and it is shown fine on my end.
Are you still unable to see your form in Chrome? It was probably temporary issue. If your form is still now showing in Chrome please provide us ID of your form. If you have embedded your form to website, please provide us URL of your website so that we can test your form there.
You can also try to update your Chrome to latest version.
