Neither of my two forms display correctly

In preview, they look correct but on my website neither form is complete.

That is because of the mobile responsive widget as well as your form container. Removing the widget should fix the problem. You can enable the responsive in the form preferences or in the designer and that should do the responsive thing. Either one will work, I've fixed this for you.

The form is cut off at the bottom, you need to re-embed it and make sure to include everything. Or, adjust the height manually in the current codes:
