For the name entry i only want one box.

In Asia with different ethnic groups the presence of 3 boxes for NAME to indicate

FIRST NAME

MIDDLE NAME AND 

LAST NAME 

could be confusing.

It is easier leaving one box with a longer width

Instead of using a FULLNAME field, you can use a regular TEXTBOX field to enter names

This will accomplish the same as a fullname.
