Is there any reason for this?

It happens a lot. I have to down tools, clear cache, clear everything and start again.
Is it something I am doing wrong?
Are there any other users experiencing the same thing?

I assume this was your jotform http://www.jotformpro.com/form/53617157606963 that you were referring to be having the problem.
I test the form on my browser but so far I could not reproduce the same issue yet.
I checked the form and I have noted that it was a very long form and contains a lot of Conditions on it.
base on the screenshot image you had provided, it seems that the page resources were just not able to completely load.
This is possible to happen if the network/internet is timing out before it was able to properly load the page resources.
As of this time, no other user have reported the same occurence. But it is not good to use the other user's as basis for any issue since most users have different types of forms. (i.e no other users have the same form as you have now)
If the issue persist and consistently happens, may I also suggest a little redesign of the form and at least reduce the number of Conditions on the form. This should help in decreasing the bandwidth needed to complete load the form evertime it was loaded on the browser.
