HELP -- form is collapsed -- URGENT

Hello,

When I preview my form all of a sudden it is completely collapsed.  I was experimenting with Adobe Document Sign widget and that is when the problem started I have since removed the widget but it is still happening !!!!

I checked your form but it is disabled, so I am unable to preview it. Please enable your form so that we can check if the problem is still present.

May we also know if the form is embedded on a website when you viewed it? If the problem persist in your end, perhaps a screenshot would help us better to identify the problem.

Another thing that you can check, if you have plugins/addons/extensions in your browsers like Adblock applications, then that's one possible reason whey scripts or resources of the form is not being loaded, giving you a "collapse" form. We'll wait for your response.
