How can I get access to it to clone a form into my account?

Just read that post and I see that you've a test form 

http://form.jotformpro.com/form/52856272139966   doing exactly what I'm looking at.

How can I get access to it to clone it into my account  (account : eflaefla) ?  so that I can take it as starting point for the form that I want to create.
