How do we change information on downloaded PDF?

There is a lot of information NOT included in the PDF version of the jot once downloaded.  We need ALL of the information that was entered.

While viewing your submissions, there is an option to include headers and text in your submissions:

From that same location, you can choose which fields to include in the PDF output.
