I need to add a Form Cover. One of the videos suggests that there is an ADD Form Cover Button, but I don't see it.

I need to add a form cover and I don't see an ADD Form Cover Button. Please advise

Simply go to the Form Designer, and click directly on the Form Cover tab, then upload the image, and it will be setup.

You can position the image as you want to, with the "Position" and "Top position" options:
