putting a waiver on a form

    I want to put a waiver on the front of my form and then if they agree with the waiver it brings them to the area to start filling out the form and if they select no it brings them our company website.  what is the best way to do this ?

    You will need to add the HTML element to your website and then display it over the website.

    If you want to do it when loading the link to your form, then it's not possible, you will need to embed it in your website.
