I've been notified by users that they are getting a "form over quota" error message. Upon further checking I realized that we apparently exceeded our 10 payment submissions... Could you please let me know if this is a per month restriction or per account or form?

The submission counter counts the number of submissions in your account as a whole. So it is the total number of submissions for all of the forms.
The submission limit is per month. The counter will reset back to 0 every first day of the month.
I can see here that you upgraded to Bronze package. I was able to see your forms properly now.
