I can no longer see my submissions names on the downloaded form. Please help

I last downladed an excel sheet with our event participants on 2/15. Now, 2 days later, the names are gone/ I cannot view them. Please any assitance would be greatly appreciated. 

Could you please share us the link/name of the form with this issue? 

I have checked one of your forms and when I downloaded the Excel report I was able to see all the data.

Once we have the name or link to your form we will make further tests.
