    I am not sure which of our apps you are referring to as the "pages" app:

    When it comes to uploading an image in order for it to be displayed on your form, please try using the Image field. Upon adding the Image field, it will allow you to select from one of the already uploaded images, or to upload a new image to be displayed on your form:
